Long-Term Services and Supports Coordinator (LTSSC) to work as a member of the One Care team to identify and monitor clients for an all-inclusive care program. LTSSC will conduct home visits when needed, implement services through One Care providers, manage service authorizations and meet regularly with an interdisciplinary team to develop and monitor care plan. Provide support to One Care Unit with tasks including but not limited to purchased services, authorizations and other administrative tasks as identified and needed.
Applicant must be able to multitask, be organized, possess good writing and communication skills, and have ability to work independently and as a member of an interdisciplinary team. Training provided. Knowledge and skills to work successfully in an interdisciplinary team required. Use of a reliable vehicle including active vehicle insurance with a current license to drive required. Ability to write an Individualized Care Plan, document clearly and communicate effectively, verbally and in writing, across complicated service and support systems preferred. Strong computer skills with experience working with Windows based software application required. Excellent communication, documentation, time management, and organizational skills required.
The Long-Term Services and Supports Coordinator (LTSSC) coordinates support services as part of the One Care Primary Care Team. The LTSSC’s basic function is to assist those who are eligible to obtain services which will enhance their quality of life and allow them to live as independently as possible with dignity and respect. Help people with disabilities meet their daily needs for assistance and improve the quality of their lives. Examples include, but are not limited to, adult day health, adult foster care, day habilitation, durable medical equipment, group adult foster care, home health, therapies, personal care attendant services, as well as support for everyday tasks such as laundry, shopping, and transportation. Provide support to One Care Unit with tasks including but not limited to purchased services, authorizations and other administrative tasks as identified and needed.
Long-Term Services and Supports Coordinator (LTSSC)-social services-full & part time
Bachelor’s degree in Social Work or Human Services preferred or at least two years’ experience required working in a human services field with people with disabilities, behavioral health needs, or individuals in need of LTSS between the ages of 21-64.
A hiring bonus will be awarded in 2 parts, half at 120 Days, and half at 1 year. The bonus for this position is $1,000 (full time).
