Licensed Practical Nurse (LPN)

The primary purpose of your job position is to promote and restore resident’s health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to residents, friends, and families; supervising assigned team members. You receive administrative supervision from the Director of Nurses, Assistant Director of Nurses, and Nurse Mangers. Works rotating shifts, holidays and weekends as scheduled. Initiates emergency support measures, i.e. CPR, protecting residents from injury, etc. DELEGATION OF AUTHORITY As the Licensed Practical Nurse, you are delegated the responsibility and accountability necessary for carrying out your assigned duties. ESSENTIAL JOB FUNCTIONS: RESPONSIBILITIES AND STANDARDS

  • Identifies resident service requirements by establishing personal rapport with potential and actual residents and other persons in a position to understand service requirements.
  • Established a compassionate environment By Providing emotional, psychological, and spiritual support to residents, friends, and families; respecting the resident’s dignity and privacy consistently.
  • Promotes resident’s independence By Establishing resident care goals; teaching resident, friends, and family to understand condition, medications, and self-care skills; answering questions.
  • Assures quality of care By Adhering to therapeutic standards; measuring health outcomes against resident care goals and standards; making or recommending necessary adjustments; following facility and nursing department’s philosophies and standards of care set by state board of nursing; State Nurse Practice Act, and other governing agency regulations.
  • Resolves resident problems and needs By Utilizing multidisciplinary team strategies; ensuring a timely response.
  • Maintains the stability and reputation of the health care facility By Complying with accreditation requirements; projecting a personal and professional image when dealing with people external to the facility.
  • Maintains safe and clean working environment By Complying with procedures, rules, and regulations; calling for assistance from health care support personnel.
  • Protects residents and employees By Adhering to infection control policies and protocols, medication administration and storage procedures, and controlled substances regulations.
  • Documents resident care services By Charting in residents’ health record(s); following documentation policies, procedures, and protocols of the facility; ensuring accuracy of admission and routine resident care observations/transfer notes which accurately reflect the resident’s clinical status; ensuring all residents admitted have a Comprehensive Admission Nursing Assessment completed; assessing the resident and documenting identified changes in the physical or psychological condition of the resident; contributing to the formulation and review of nursing care on assigned residents weekly and/or PRN; completing multidisciplinary Discharge Summaries, transfer papers, etc. according to policy; ensuring all forms have proper resident identification, allergies listed, approved facility abbreviations utilized, timely completion of vital signs, timely progress notes with full signature and title, flow sheets completed according to policy; charting dates when medications are stated and stopped; charted according to proper policy; taking appropriate vital signs when administered medications; charting when residents do not take their medications and circling same on medication administration records; following the procedure for narcotics documentation; charting parental fluids and all additives per policy; following the procedure for starting Ivs and the required documentation per policy; charting any adverse reaction(s) to medications or Ivs; utilizing only facility approved abbreviations; monitoring IV sites and changing catheters according to facility policies and procedures.
  • Maintains continuity among nursing teams By Documenting and communicating actions, irregularities, and continuing needs; assigning equitable care assignments appropriate to resident needs at the beginning of the shift; reporting staffing needs to Director of Nurses, Assistant Director of Nurses and/or Nurse Manager.
  • Maintains residents’ confidentiality and protects operations By Keeping information confidential; following release of information protocols; following state laws and federal regulations regarding the right to access and release of health information; ensuring that nursing documentation is informative, descriptive, and timely as required by all requirements and/or regulations as well as the resident’s response to care.
  • Ensures operation of equipment By Completing preventative maintenance requirement; following manufacturer’s instructions; troubleshooting malfunctions; calling for repairs; maintaining equipment inventories; evaluating new equipment and techniques.
