Job Details
Responsibilities
- Provide case management services to patients with HIV, including coordinating care and counseling patients and their families.
- Conduct triage assessments and check vitals, administer medication, and perform blood work as needed.
- Coordinate patient care with other healthcare professionals, including physicians, social workers, and other healthcare providers.
- Manage and update patient charts and records, ensuring that all information is accurate and up-to-date.
- Communicate with patients and their families, providing education and resources to help them manage their condition.
- Collaborate with other healthcare professionals to develop and implement care plans that meet the individual needs of each patient.
- Provide support and guidance to patients and their families, helping them navigate the healthcare system and access the resources they need to manage their condition.
Qualifications
- New York State Licensed RN with at least 1+ years of experience in clinical nursing duties.
- Strong organizational skills, with the ability to manage multiple tasks and priorities simultaneously.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Bi-lingual skills are a plus.
- Strong charting skills, with the ability to maintain accurate and up-to-date patient records.
- Ability to work independently and remotely, with strong attention to detail and the ability to manage time effectively.
