Registered Nurse- Home Health

Community Health Network (CHN) has been placing experienced nurses and physical therapists in home care, hospice, and case management for over 30 years. We offer flexible work options and outside of the box opportunities that focus on your needs and interests.

We are currently assisting one of our top home health agencies and have a immediate needs for RN’s! Full Time – Home Health RN to do Medicare home health skilled visits. Current territories available:

Waltham/Woburn/Reading/N Reading/Wakefield, MA area.

Arlington/Medford/Malden/Lexington/Burlington, MA area.

The Home Care Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN’s and Home Health Aides.

Full-time, Monday-Friday.

No weekends, on-call or holidays required!

Great pay rate!

*** Requires a minimum of 1 year Home Care RN Case Management experience with a Medicare certified agency. ***
