Case Manager

Urban League of Eastern Massachusetts

This is a Contract position in Roxbury, MA posted November 21, 2021.

The Case Manager is responsible for monitoring, supporting, and enhancing participants’ success in the program through assessing external risk factors and intervening as needed to support participant program completion. This includes direct support of participants by providing access to external resources for delivering counseling and related social services. This position will also collaborate with the admissions, workforce development programs, and internships by delivering life skills programming and coaching.

Duties:

Direct Services

  • Connect participants to appropriate available resources and help them apply for assistance programs
  • Develop and maintain a “service plan” and records for those participants who need “case management” services
  • Provide one-on-one brief counseling and support as needed and appropriate for participants
  • Conduct participants assessments and evaluations while providing appropriate follow-up services
  • Identify participant needs that can be addressed through group work, and run supportive, or psycho-educational groups, as needed
  • Develop close, supportive, and trusting relationships with participants, to make one’s self-available as a tertiary level of support for all participants
  • Provide support to staff as they work with participants who need social work support identified through the admissions process or in the course of their involvement in the ULEM program
  • Build relationships with participants both through individual meetings/sessions and being visible during lunch, breaks, and in the classroom, when appropriate
  • Attend WFD meetings to discuss the progress of participants, and to develop interventions and direction, as appropriate

Program Development

  • Build new organizational relationships with community organizations and social service agencies around mental health, housing, pregnancy/parenting, financial services, family issues, substance abuse, etc. so that they are available to participants as needed
  • Develop an integrated Life Skills curriculum that is directly applicable to the participants’ lives. (i.e. anger management, sexual harassment, life transitions, and pressures, etc.)
  • Add to and update the ULEM Resource Guide for participant

Intake

  • Work closely with the admission process by conducting one-on-one interviews and assessments of prospective participants
  • Manage follow-up meetings with prospective participants who are in need of additional resources such as stable child care and substance abuse counseling and etc.

Education and Experience:

  • Bachelor’s Degree required Master of Social Work / Psychology / Mental Health degree from an accredited university/college preferred
  • 2 years of experience as a counselor/therapist for adults
  • Experience working with diverse populations required
  • Experience developing, implementing, monitoring, and measuring the effectiveness of counseling and related programs
  • An existing network of contacts within and extensive knowledge of the social service’s resources available in the Metropolitan Boston area
  • History of cultivating relationships and building alliances with agencies and schools
  • Commitment to diversity and inclusion
  • Experience developing and implementing related curriculum
  • An engaging and collaborative leadership style with mature interpersonal skills and the ability to juggle and manage multiple, competing priorities and deadlines
  • Preferred ACT WorkKeys ® Silver National Career Readiness Certificate

Job Type: Full-time

Pay: $55,000.00 – $65,000.00 per year
